Three things, in order of cost. Syntax: is the address even well-formed. Domain: does the domain exist and publish mail records that could receive anything. Mailbox: does that specific address appear to exist on that server. The first two are definitive; the third is a best effort, because many mail servers deliberately refuse to confirm or deny a single address.
A catch-all domain accepts mail for every address at that domain, real or not, so a verification probe gets a "yes" that means nothing. Marking those addresses deliverable would be a lie of convenience. Leads Ranger flags them risky so the decision is yours: many catch-all addresses are perfectly good, they just cannot be proven from outside.
Under 2% is the usual comfort zone, and under 1% is where you want to live. Above roughly 5%, mailbox providers start treating your sending as careless, and the penalty lands on your inbox placement rather than showing up as an error you can see. That is what makes bounces so expensive: the cost is paid by the mail that did not bounce.
Business email data decays at roughly 20 to 30% a year as people change jobs and domains lapse, so any list older than a few months deserves a re-run before a campaign. Leads Ranger reduces how often this matters by checking again at send time, but a list you have been sitting on is still worth verifying deliberately.

No, and anyone claiming otherwise is selling something. A mailbox can be full, a server can be misconfigured, and an address can be deleted between the check and the send. Verification removes the large, predictable share of bounces, which is enough to keep your reputation intact; the residue is normal and every sender has it.
